I think you're asking about how it affects the shoulder joint (impingement, a/c joint problems, etc.). I've found that whenever flat bench has hurt, I've been able to continue with decline bench pain-free. If you've been having shoulder problems, ask a physiotherapist. If you get one that tells you not to lift, find a new one. I found one that used to be a physio for rugby, so she knew I'd be lifting anyway. She suggested neutral grip dumbbell presses, and having my arms about 60 degrees away from my body instead of 90.
