Predominantly vascular damage. If you fully restrict blood flow, then by training increase blood pressure and output, you can cause damage to the vascularity of the occluded area.
One of my class mates is doing occlusion training for her thesis research, if you want, when I see her next week, I'll get some more in depth studies and medical reviews regarding the application and use of occlusion.
