Indoor, i find a treadmill the easiest to do HIIT on, but that can get somewhat dangerous. I just set up the standard 1:1 interval setting, and when the run portion comes up, i set the incline really high, like 7-8, and run at a pretty fast pace, usually around 9-10. If you get tired or feel you cant keep up, i just jump off to one side. After 30 seconds of actual sprinting and it not going back down, i usually jump off to one side and wait.
I usually have the slow portion around 2-3 with no incline.
If you have ellipticals that have hand controls (like, where you hold onto the handles, some have controls right at your finger tips), i like using those to do my HIIT as i can quickly increase the resistance and stride length, or lower it.
How long depends on your conditioning, but i find anything longer than 20 minutes and your intensity level usually takes a steep drop. I would only do this on off days, once a day. Its pretty brutal on the joints/ligaments, and the body usually gets a great core workout if on a treadmill.
