1. prolly too much synthol under the lateral head
2. he could just be injecting large amounts of test into his shoulders like valentino did with his arms and delts. (he injected 10ml of test into each bicep at a time, sure there was some synthol use, but imagine injecting 10ml of test at a time)

Nah, it's not Synthol... I've seen that exact same thing before with a guy at my gym. It's just injecting a lot of test into the delts. I don't think the oil is dispersing.

Looks bad and he'll probably end up with an abcess.
